An initiating visit is a qualifying face-to-face encounter that Medicare requires before CCM begins for a new patient or a patient not seen by the billing practitioner within the previous year. CMS identifies qualifying E/M visits, Annual Wellness Visits, and Initial Preventive Physical Examinations as potential initiating encounters.
