A Claim Adjustment Reason Code (CARC) is a standardized X12 code used to explain why a payer adjusted, reduced, or denied payment on a processed claim or service line. CARCs help billing staff understand differences between billed and paid amounts, although they do not always identify the underlying cause of the adjustment.
