A HIPAA breach is generally an impermissible acquisition, access, use, or disclosure of protected health information that compromises its security or privacy. An impermissible use or disclosure is presumed to be a breach unless an applicable exception applies or the required assessment demonstrates a low probability that the PHI was compromised.
