A rejected claim is a medical claim that fails validation checks and is returned for correction before it can be processed further. Rejected claims commonly result from missing information, demographic mismatches, coding errors, formatting issues, or enrollment problems. Unlike a denied claim, a rejected claim has not completed the payer’s review process and must usually be corrected and resubmitted.
